Cancellations

As a courtesy to our clients and practice, please provide a minimum of 24 hours notice should you need to cancel or reschedule an appointment. We kindly ask that you cancel your appointment via text or by phone more than 24 hours in advance to avoid the cancellation fee. Your signature on this form indicates your agreement to comply with this policy. You will be charged a $150 cancellation fee if an appointment is canceled less than 24 hours in advance or if there is a no-show.
